You may now download all the forms you need for your first dental visit with us. Completing them at home will make it easier to provide all the information, and save valuable time during your visit!

Following are forms you will need:

Every adult patient and every guardian of a minor patient needs a Notice of Privacy Practices. The federal government requires that you read this form, but you need not return it to us.
After reading this form, you will need to read and sign Acknowledgement of Privacy Practices.

Every patient needs a Patient Registration.
Every adult needs a Medical History.
Every child needs a Child History.
Each insured adult needs an Insurance Agreement.
